Want us to let you know about placement vacancies?
We welcome referrals from Local Authorities, NHS services, Placement Teams, and other professionals looking for Supported Living placements.
We also accept private referrals, but find that most people who would benefit from supported living qualify for local authority funding. We recommend getting in touch with your Local Authority if you think that may apply to your situation.
Our Referral Process
Our team responds quickly and works in partnership with you from day one.
Initial Contact
We accept referral enquiries via online form, phone or email.
Referral Review
Our team reviews the referral within 48 hours.
Assessment
We meet the person in their current setting, where possible, and assess how we can meet their needs.
Care Plan & Proposal
We share details of a proposed property, tailored care plan and associated costs with the commissioning or funding body.
Transition Planning
If agreed, we begin staff matching, shadowing and — where need is identified — additional training to prepare for a safe transition.

Our team supports the move into an exciting new home!
