Families & Individuals requiring support

  • We support adults (18+) with complex care and support needs, including learning disabilities, autism, mental health conditions, behaviours that may be deemed challenging, histories of trauma, and a wide range of diagnoses.

  • Supported Living means having your own home, with support built around you. The level and type of support are personalised, helping people live as independently as possible while feeling safe, respected and supported.

  • Support is shaped around each person’s needs, routines and goals. This might include help with daily living, emotional wellbeing, health, building skills, and staying connected to family, friends and the community.

  • The people we support hold their own tenancy, which supports independence, stability and choice. Depending on individual circumstances this may be in a shared or solo-occupancy home.

  • Absolutely. Families are involved as much or as little as the person wishes. We value family relationships and aim to keep communication open, respectful and consistent.

  • Yes. This is their home. We actively support meaningful contact and visits, in line with the person’s wishes.

  • We offer different types of Supported Living, depending on need:

    • Shared homes with others

    • Self-contained flats with nearby staff support

    • Single-person homes with higher levels of staffing

  • We support people to build ordinary, meaningful lives — taking part in activities they enjoy, developing confidence, forming relationships, and feeling part of their local area.

  • Yes. We are experienced in supporting people who need higher levels of support, including 1:1, 2:1 or 3:1 staffing. Support levels are carefully planned around the person’s needs and reviewed regularly, so they feel safe, understood and well supported.

  • Safety is managed through personalised risk planning, trauma-informed support, well-trained staff, and close working with health and social care professionals. Plans are reviewed regularly.

  • We encourage open conversations and early discussions. Families can raise concerns with the local team or management, and we also have a clear complaints process if needed.

Commissioners & Brokerage teams

  • We support adults (18+) with complex care needs, including learning disabilities, autism, mental health conditions, behaviours that may be deemed challenging, trauma histories, and co-occurring diagnoses.

  • Yes. We have experience supporting people stepping down from hospital or secure settings, as well as individuals whose previous placements have broken down. All referrals are assessed on an individual basis.

  • We currently operate across London and the Home Counties. Through strong partnerships, we can also develop new services beyond these areas where appropriate.

  • We provide flexible Supported Living models, including:

    • Shared care homes

    • Self-contained placements with staff hubs

    • Bespoke and specialist single-person placements with high staffing ratios

  • Staffing is based on assessed need and can range from low-level support to 1:1, 2:1 or 3:1 packages, including waking night or sleep-in support.

  • We review referrals within 48 hours.

  • Our process includes initial contact, referral review, assessment, care plan and costings, and structured transition planning with staff matching and training.

  • This typically includes assessments, risk information, behaviour support plans, health reports, safeguarding details, and an overview of needs, goals and preferences.

  • Yes. We work closely with social workers, commissioners, health professionals, therapists, housing providers, advocates and families, prioritising clear communication and shared decision-making.

  • Risk is managed through personalised assessments, positive behaviour support, trauma-informed practice and regular review. Restrictive practices are used only when absolutely necessary and always within legal and best-practice frameworks.

  • Yes. Abacus is registered with the Care Quality Commission (CQC) to provide personal care.
